Terms & Conditions

1.0 AUTHORIZATION TO PARTICIPATE:

I, the parent or legal guardian of the child named in this registration, grant authorization for my child to participate in youth baseball/softball activities sponsored and/or sanctioned by Charter Oak Youth Baseball and Softball Association (Hereinafter referred to as COYBSA).  COYBSA assumes no risk or hazards incidental to such participation and the parent or legal guardian of said child does hereby agree to hold harmless COYBSA, its officers, sponsors, agents, participants, associated volunteer personnel, umpires, Charter Oak Unified School District, its officers and agents, and persons transporting the minor to/from sponsored and/or sanctioned activities from any claim, including the payment of all damages, expenses, and costs (including reasonable attorney fees) associated with such claim.

2.0 EMERGENCY AUTHORIZATION:

In the event of accident or emergency involving the minor child named in this registration, I grant and authorize managers, coaches, and/or officers of COYBSA or their designated representatives to seek and obtain medical aid, treatment, and/or emergency care deemed necessary as a consequence of any participation in, attendance at, or travel to or from scheduled or authorized events of COYBSA.  I further agree to hold harmless COYBSA, its officers, sponsors, agents, umpires, and affiliated volunteer personnel from any and all claims which may arise as a result of any such incident, including the payment of all damages, expenses, and costs (including reasonable attorney fees). Should treatment be necessary, it is understood and agreed that any medical insurance of said child may be covered by other than that provided by COYBSA will be considered the primary insurer.  If medical insurance provided by COYBSA is utilized, responsibility for the payment of deductible fees associated with such shall be assumed by the parent(s) or legal guardian(s) of the child named on this registration.

3.0 RETURNED CHECK POLICY:

A returned check charge of twenty-five dollars ($25.00) shall be assessed on all checks returned to COYBSA unpaid by your bank.

4.0 SPONSORS REQUIREMENT:

Each player is required to turn in one (1) case of (variety pack) 20 oz. Gatorade and one (1) case of 16 oz. water bottles to the Sponsor Director per the Sponsor Director's requested schedule. If a team does not complete their donate at that time, they will be fined $200 and have their games suspended until delivery.

5.0 ALL-STAR PARTICIPATION

All-star team participants and/or their legal guardians, determined at the time of registration, hereby agree to pay the cost of participation for the All-star season, as determined by COYBSA and its Board of Directors, prior to start of official practices for said season. The payment, which will be determined based on tournament fees and equipment costs each season, will cover the full cost of the following items:

• SOFTBALL EQUIPMENT: Uniform (incl. Jersey, Visor, Socks), Helmet, and Bat Bag (optional as determined by COYBSA).
• SOFTBALL TOURNAMENTS: 5 tournaments, to be determined by the Director of Softball, will be provided (typically these consist of Orange Canyon, PYL, Fountain Valley, Districts, and Chino Milkcan Classic). In addition to these, a COYBSA tournament may be held.
  If a team places 1st or 2nd at Districts, COYBSA will pay for its entry fee to the State Tournament.
  If a team places from 1st to 4th at the State Tournament, COYBSA will pay for its entry fee to Nationals.
• BASEBALL EQUIPMENT: Uniform (incl. Jersey, Pants, Hat, Socks), Helmet and Bat Bag (optional as determined by COYBSA).
• BASEBALL TOURNAMENTS: 4 tournaments, to be determined by the Director of Baseball, plus Sanctions will be provided. In addition to these, a COYBSA tournament may be held. Sanctions include District, Area, Regional, Super Regional, and the World Series, should a team advance as such.
• NOTE: For both softball and baseball, if a COYBSA tournament is not conducted, each team can decide to find another tournament to play at their own cost. If a COYBSA tournament is held, PARENTS WILL BE REQUIRED TO DONATE THEIR TIME to the event.

6.0 PHOTO AND VIDEO RELEASE

Charter Oak Youth Baseball and Softball Association (COYBSA) retains the irrevocable and unrestricted right to take, use, and publish photographs or other images of all players, parents, and other participants of COYBSA events, with or without identification, in any print, electronic, digital or other media; and to alter the same without restriction for any lawful purpose, including, but not limited to, such purposes as publicity, illustration, advertising, and Web content. Participation by all individuals associated with COYBSA is voluntary and COYBSA will not provide financial compensation of any type associated with the taking or publication of these images. COYBSA will maintain irrevocable rights to these images and their uses in perpetuity. COYBSA and its legal representatives shall not be held liable for any claims relating to said images.
